Acknowledgment of Risk: I, , understand that participating in Brazilian Jiu Jitsu involves physical contact and strenuous physical activity, which may result in injury. I acknowledge that I am voluntarily participating in this activity with knowledge of the risks involved.
Release of Liability: In consideration of being allowed to participate in Brazilian Jiu Jitsu classes and activities, I hereby release, waive, discharge, and covenant not to sue Alexander Soutar (MKM BJJ LIMITED), its owners, instructors, employees, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, while participating in such activities, or while on the premises where the activities are conducted.
Health & Fitness: I certify that I am physically fit and have no medical condition that would prevent my full participation in Brazilian Jiu Jitsu. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in these activities.
Indemnification: I agree to indemnify and hold harmless Alexander Soutar (MKM BJJ LIMITED), from any loss, liability, damage, or costs, including court costs and attorney’s fees, that they may incur due to my participation in Brazilian Jiu Jitsu, whether caused by negligence or otherwise.
Photography/Video Release: I grant permission to Alexander Soutar (MKM BJJ LIMITED), to use photographs and/or video recordings of me for promotional purposes, including but not limited to social media, websites, and other marketing materials.
Severability: If any portion of this waiver is found to be invalid or unenforceable, the remaining portions shall remain in full force and effect.
Acknowledgment of Understanding: I have read this waiver of liability, assumption of risk, and indemnity agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
